Jouw nieuwe baan
Als Eerst Verantwoordelijke Contactverzorgende (EVC/EVV) binnen Zuyderland Zorgcentra begeleid je de intake van nieuwe cliënten, inventariseer en bespreek je de zorgbehoefte, wensen en mogelijkheden met cliënten en hun familie. Je coördineert het proces op basis van het zorgleefplan, organiseert evaluaties en borgt de uitvoering. Daarnaast voer je reguliere werkzaamheden als Verzorgende IG uit.
