In the home care sector, you deliver high-quality care to clients in the community: you assess care needs and assign appropriate services. You predominantly work independently but also as part of a team, coordinating care. As a district nurse, you work with diverse groups, from the elderly and chronically ill to individuals with disabilities or those in palliative care. You fulfill a coaching and leadership role within the team: you possess extensive knowledge of social services, guide colleagues through complex issues, and encourage a learning and reflective attitude within the team. Your role is not only focused on providing care to clients but also on supporting and developing your team members.

Requirements

• Completed HBO Bachelor’s degree in Nursing (HBO-V).
• Valid BIG registration as a nurse.
• Skills in clinical reasoning, assessing care needs, coaching, and collaboration.
• Strong communication skills and the ability to build trust with clients and their families.
• Ability to work independently and take responsibility for care delivery to clients in the community.

Responsibilities

• Perform nursing procedures at clients' homes, such as wound care, medication administration, and palliative care.
• Use clinical reasoning to make decisions about client care based on clinical data and scientific knowledge.
• Coach and support the care team and assist colleagues in the community.
• Assess and assign care based on needs and develop and evaluate care plans.
• Participate in projects aimed at improving care processes and enhancing the quality of home care.
• Maintain close communication with general practitioners, informal caregivers, and other stakeholders in the community.
