Werken met een diverse doelgroep, schakelen in complexe situaties en denken in mogelijkheden. Als casemanager binnen het forensisch ACT-team krijg je te maken met uiteenlopende uitdagingen waarbij doorzettingsvermogen, flexibiliteit en stressbestendigheid onmisbaar zijn. Je houdt het hoofd koel wanneer de situatie daarom vraagt en zoekt samen met cliënten en collega’s naar passende oplossingen. Voor het forensisch ACT-team zoeken wij een casemanager voor 32–36 uur per week. Iets voor jou?
Waar ga je werken?
Het Forensisch ACT-team biedt intensieve, outreachende behandeling en begeleiding aan mensen met complexe psychiatrische problemen die strafbaar of risicovol gedrag vertonen of hebben vertoond. Vaak is sprake van meerdere uitdagingen tegelijk, zoals ernstige psychiatrische problematiek, verslaving, persoonlijkheidsproblematiek, maatschappelijke ontregeling en problemen op het gebied van wonen, werk, financiën en sociale relaties.
Onze cliënten bevinden zich op het snijvlak van zorg en veiligheid. Sommigen wonen zelfstandig, anderen verblijven binnen een beschermde woonvorm of maken deel uit van een intensief samenwerkingsverband zoals de levensloopaanpak. Zonder passende begeleiding en behandeling bestaat een verhoogd risico op maatschappelijke uitval, crisis of recidive. Juist daarom maakt jouw inzet een zichtbaar verschil, zowel voor de cliënt als voor de samenleving.
Je komt terecht in een ervaren en betrokken multidisciplinair team van ruim vijftien professionals, bestaande uit een psychiater, Gz-psycholoog, sociaal psychiatrisch verpleegkundigen (SPV), sociaal juridisch dienstverleners (SJD), een IPS-jobcoach en casemanagers. Vanuit een gedeelde caseload wordt intensief samengewerkt aan herstel, stabilisatie en risicobeheersing. De lijnen zijn kort, collega's staan voor elkaar klaar en er is veel ruimte voor eigen initiatief, professionele ontwikkeling en inhoudelijke uitwisseling.
Het Forensisch ACT-team werkt nauw samen met de forensische polikliniek, forensische woonvoorzieningen, reguliere GGZ-teams, gemeenten, politie, reclassering, veiligheidshuizen en andere partners binnen de zorg- en veiligheidsketen. Hierdoor werk je niet alleen vanuit één locatie, maar ben je actief in de gehele regio en beweeg je je binnen een breed netwerk van professionals rondom de cliënt.
